A Care Manager can arrange for a variety of services for a senior or their family, saving time and money and reducing stress. A Care Manager’s duties are similar to a hospital discharge planner. However, unlike hospital services that end at the time of discharge, we stay with our clients as long as they need. - Medical Expertise: Home Care agencies are licensed to provide non-medical care, called “custodial services”. At some time or another in the aging process, medical care is often needed. At this point the home care agency is required to refer the client to a licensed home health care agency or relocate the client to a nursing home or assisted living facility. Change can be stressful to the client. We have nurses on staff that can provide medical expertise when needed. We also coordinate and refer to local medical therapies that may be paid for by Medicare when skilled professionals such as physical therapists, IV therapists, RN’s or occupational therapists are needed in the home.
